Patient Attestation:

Patients in the Consumer Directed Program can attest the completed visits on a weekly basis in the Patient Portal.

Patient Requirements:

• Last Name

• Date of Birth

• Cell Phone to receive an authentication code (ensure privacy and security).

  • If the patient does not have a cell phone, a designated rep should be identified and that individual can receive the code for access.
